Wakefield Council is commissioning for the provision of the Preventing Rough Sleeping Model (PRSM) from 1 October 2015 for an initial period of 18 months. The Service will consist of: - Coordination of the Rough Sleeping Model in Wakefield - Street outreach to verify people as rough sleeping - A Hub facility where people can stay overnight in the short term when being supported through the model - Arranging of move on accommodation from the Hub. The successful tenderer will need to have, or have identified, a suitable location for The Hub function of the service.
