NHS Scarborough and Ryedale CCG in collaboration with NHS Vale of York CCG and NHS East Riding of Yorkshire CCG are seeking expressions of interest from providers of Neurology Services

The provider will deliver a range of general and specialist neurological outpatient/community based clinical services for patients within North and East Yorkshire.

The Services will include provision for:

  • Cognitive Disorders
  • Epilepsy
  • Headache and Migraine
  • Neuromuscular Disorders
  • Parkinson’s Disease
  • MS

The overall aim of the service is to improve the health and wellbeing of the population living with neurological conditions and to provide a patient centred service in collaboration with other relevant services.

The service will achieve the following outcomes:

  • Provide optimum clinical support for primary care to enable joint coordination of care for the specified population by:  Working collaboratively with primary care

 Providing specialist advice, support and guidance

 Providing an education programme

  • Ensure patients have access to the appropriate professional in the appropriate setting at the right time
  • To support seamless transition between services
  • To ensure pathways are followed, within NICE recommendations, and variances reviewed and actioned
  • To allow ongoing access for the GP for advice, support and guidance
  • To provide follow up access for patients via defined self-referral criteria

To ensure people are supported to manage their condition to maximise their quality of life, at every age and every stage by:

  • Ensuring patients are provided with a clinical management plan
  • Providing advice, health education and self-management strategies to patients
  • Ensuring patients have regular reviews

To reduce the rate of preventable admissions by:

  • Ensuring patients are supported to follow their clinical management plans
  • Providing neurological education for primary care, paramedics and residential care
  • Providing ongoing specialist advice, support and guidance to primary care
  • Ensuring access to neurology specialists
  • Reducing the time spent in hospital

To maximise integration opportunities with social care and the third/voluntary sector by:

  • Working collaboratively with referrers and supporting services including MDT, social care and voluntary services
  • Ensuring third/voluntary sector support is offered at all stages of the pathway and timely referral is made where appropriate/accepted by patient

Additional information: NHS Scarborough and Ryedale CCG in collaboration with NHS Vale of York CCG and NHS East Riding of Yorkshire CCG are inviting Providers to share their views and experience of Neurology Services.

All Providers are invited to participate including providers currently in the Neurology Services market or considering a move into the market within the geographic region.

The views of providers will be highly valued and will be used to inform the service specification and procurement processes.
