NHS Hartlepool and Stockton-on-Tees Clinical Commissioning Group (CCG) propose to commission an integrated urgent care service which will include provision of Out of Hours Primary Care Services, Minor Injuries Unit Services and Walk In (urgent care) Primary Care Services for the population of Hartlepool and Stockton-on-Tees.
In practice, this means bringing together the current urgent care services to ensure our patients are quickly directed to the right care, minimising disruption and frustration, improving efficiency, and reducing extended waiting times.
The objectives are to ensure:
| • | Services are joined up, seamless and coordinated |
| • | Delivery of an evidence based approach to commissioning services |
| • | Patients are not confused of what to do, who to call and where to go |
| • | Services are safe, responsive and of a high quality |
| • | Active involvement of patients and public in service improvements |
| • | The effectiveness and outcomes of urgent and emergency care services are monitored |
| • | Urgent care services are delivered 24 hours a day, 7 days a week where appropriate |
| • | Management of patients is undertaken at the point in which they present (first point of contact) |
| • | Better continuity of care |
| • | Increased awareness of early detection of illness and options for self-care |
The contracting authority will develop the service model to incorporate the Public Services (Social Value) Act 2012, specifically:
| • | How what is proposed to be procured might improve the economic, social and environmental well-being of the relevant area. (Area is the geographic area for which the procurement is taking place) |
| • | How, in conducting the process of procurement, it might act with a view to securing the improvement. |
