NHS North East London Integrated Care Board is seeking to commission Tower Hamlets Self-Management Support Service. This is a Cardiovascular Disease (CVD), Chronic Kidney Disease (CKD) Hypertension and Diabetes Self-Management Support Service. The provision of essential support services aimed at empowering individuals living with these chronic conditions to actively manage their health and well-being.
The service will focus on increasing the knowledge, skills, and confidence of its users and to promote independence. The Provider will actively mitigate against the participants developing a dependency on the service by promoting independence and exploring ways for people to implement and sustain activity following programme end.
For people with type two diabetes, the programme should ultimately support people to attend X-PERT diabetes education programme via https://www.bartshealth.nhs.uk/community-diabetes-service/ as recommended by NICE guidance

The estimated total Value of the Contract is £577,725. The total value is based on an annual value of £115,545 per year. The Contract will be for an initial period of 3 years with the possibility of an extension for 2 years at the discretion of the ICB. The service is expected to commence in October 2024.

The deadline for any clarification questions is 1700hrs on 28th May 2024. All clarification questions must be submitted via the messaging facility on Atamis.
