The Primary Care Streaming Service (PCSS) will be provided from the Kettering General Hospital site located within the Emergency Department (ED)

The key requirements for the PCSS are:

Provide the right care, by the most appropriate person, in the right setting ensuring consistent responses at the first point of contact.
Streaming non-urgent patients that attend the Emergency Department back to primary care.
Primary care GP working closely with their ED colleagues to collaboratively lead the provision of Primary Care Services at KGH
Access for walk-in patients will be via the Clinical Streamer if appropriate providing a seamless pathway to any further assessment required including ED and referral (if necessary) to a hospital specialist.
Contributing to the Service development of Frail Elderly attendances at the ED from both the admission avoidance and discharge facilitation perspective.
Contributing to the development of ambulance patient access to the service.
Facilitation of unregistered patients to register with a GP practice
Providing health promotion and self-management education to patients, relatives and carers.
Assuring facilitation of 1st Assessment completion within the designated 15 minutes, including NEWS and ECG where required
Seeing and treating and discharging all patients within 4 hours of their arrival at the service
Making all see and treat decisions within 60 minutes of the patient's arrival at the service
Ensuring that necessary transfers occur within 60 minutes of arrival at the service
Assuring completion of Medway for patients attending the service, with appropriate coding
