NHS Healthcheck Service - AWARD

The Metropolitan Borough Council of Sandwell is seeking expressions of interest from suitably experienced and competent Providers of a NHS Health Check Service. The NHS Health Check Service aims to prevent heart disease, stroke, type 2 diabetes and kidney disease, and raise awareness of dementia across the population and within high risk and vulnerable groups. In April 2013, the NHS Health Check became a statutory public health service in England. Local authorities are responsible for making provision to offer an NHS Health Check to eligible individuals once every five years. The council therefore require a contractor to identify suitable clients to receive a Health Check and provide an accessible service for clients to attend where they will then receive a Health Check delivered by the contractor followed by tailored feedback for each individual on their future risk of cardiovascular disease and advice on lifestyle and referral to local lifestyle interventions as appropriate.
