Evaluation of the Nottingham City CCG Self-Care System Pilot

Nottingham City CCG seeks bids from suitably qualified suppliers to evaluate the Nottingham City Self Care Pilot.

The aims of the evaluation are to independently evaluate the effectiveness of the self-care system pilot in meeting the stated pilot aims and objectives, to highlight any areas that either positively or negatively impact on the pilot and to inform decision making and future developments in relation to ways of working, future team configuration and service delivery.

Nottingham City Clinical Commissioning Group is looking for a supplier to carry out an evaluation of the Self-Care System Pilot.

Aims and Objectives of the Evaluation

To determine whether the pilot is meeting its aims and objectives

To determine the successes of the pilot

To determine barriers to success

To determine citizen, and health and social care staff experience

To determine the extent to which health and social care professionals have implemented new ways of working

To evaluate how well the pilot operates within the wider care pathway and any unintended consequences

To identify any issues that impact on the effective implementation of the pilot

To make recommendations around existing and future ways of working, role development and service development, and City wider roll out

It is expected that the evaluation will:

Measure the impact of all the components of the pilot and whether they work effectively as a self-care system.

Evidence whether the pilot has helped to increase staff awareness of self-care support

Evidence whether staff have embraced new ways of working and whether self-care has become embedded in practice

Evidence whether the pilot has helped to increase citizen’s understanding of self-care

Evidence whether citizens feel more supported to access self-care services to support them to manage their own conditions better.

Evidence whether citizen’s report more coordinated care and a reduction in time spent in health services.

Evidence whether the pilot has helped to increased independence and well-being of citizens

Evidence whether the pilot has helped to reduce loneliness and social isolation

Evidence whether there has been a reduction in GP visits due to people feeling more able to self-care and having information and advice about how to access alternative community services Evidence whether there has been a reduction in Emergency Department attendances due to people feeling more able to self-care and having information and advice about how to access alternative services Evidence whether there has been a reduction of referrals to adult social care for people who do not meet the threshold for social care services

Include an audit of social prescriptions

Include an analysis of the outcomes identified in the outcomes framework for the pilot, including health and well-being indicators
