As part of Surrey County Council’s (SCC) commitment to keeping individuals in their own homes, rather than moving them into residential accommodation, Surrey County Council’s (SCC) Adult Social Care Service, Surrey Heath Clinical Commissioning Group (CCG) and North East Hants and Farnham CCG, also referred to as the Commissioners, wish to jointly request quotations for an Interim Hospital Home Based Care Service primarily supporting discharge of social care and continuing health care patients out of Frimley Park Hospital and Farnham Community Hospital.
