High Weald Lewes Havens Clinical Commissioning Group is considering potential routes (including procurement) to commission this service.
The specific aim of the Post Diagnostic Dementia Support Service is to be the main point of contact and provide appropriate emotional support, practical guidance and information to meet the needs of those entering the service, as well as facilitating access to well-being interventions which promote independence and quality of life. The service will be both for the person diagnosed with dementia and their family carer/supporter.
The service will work in partnership with a wide range of organisation’s including General Practitioners as part of an integrated approach. It will facilitate identifying issues leading to deterioration and will work with agencies to ensure a timely and pro-active response. It is expected that Care Coordination and Advanced Care Planning will assume a key element of the Dementia Post Diagnostic Support Service.
The Commissioner wishes to engage with service providers to gain a better understanding of their views about future ways to commission this service. Providers are asked to review the information contained in the Request for Information (RFI) document and complete the questionnaire section.
Please return your completed RFI document by no later than midday on Tuesday 15th March 2016 via the Intend Portal using the correspondence function.

This exercise is not part of any pre-qualification or selection process. It is intended as an awareness, communication and information gathering exercise offering an opportunity for all providers to help inform commissioning intentions. This process will assist the Commissioner when considering if and how procurement is an appropriate route to commission the service. A new advertisement will be published should procurement be the chosen route in the future.
