Care Home Trusted Assessor - Market Engagement

THIS IS A MARKET ENGAGEMENT STAGE

Milton Keynes City Council ('the Council') would like feedback from providers who may be interested in the contract recommissioning for our Care Home Trusted Assessor (CHTA).

The purpose and aim of the Care Home Trusted Assessor (CHTA) is to support and facilitate discharges for in-patients into care homes, and avoid discharge delays from Milton Keynes University Hospital Foundation Trust (MKUHFT).

Following a period of in-patient care, and prior to an individual's discharge to a care home, the registered manager of the care home is required to undertake a statutory assessment, or review, to establish eligibility and level of need to facility a placement and support the individual's care needs.
