The Right Care Right Here (RCRH) Partnership is Sandwell & West Birmingham CCGs vehicle of delivery for meeting our local health challenges. It is a well established partnership between the CCG, the two local authorities and other health services, formed to achieve major transformational change. Part of this focuses on expanding the level of provision of services in community settings, bringing appropriate elements of care closer to home and making more consistent the range, availability and quality of services in primary and community settings. Currently the scope and availability of Extended Minor Surgery services in Community settings is varied when comparing Sandwell to West Birmingham. The commissioning of this service will ensure consistency across the CCG footprint with all patients registered with a Sandwell & West Birmingham CCG practice being able to access the full range of Extended Minor Surgical procedures.

The Community Extended Minor Surgery service will provide a range of high quality cost effective procedures which are beyond the scope of core activities within primary care services.

The objectives are:

•High quality locally accessible services which extend choice for patients

  • Good clinical outcomes for patients
Improved patient experience of using minor surgery services
A cost-effective service that will achieve better value for money, in line with the CCG’s QIPP agenda
A reduction in inappropriate referrals to Secondary Care for diagnosis and treatment
Reduced waiting times will all patients treated within 8 weeks of referral or 12 weeks for any procedure that needs investigation.

The extended minor surgery service will offer assessment, diagnosis and treatment for a range of conditions as detailed below in accordance with relevant guidelines, pathways and evidence-based practice.

The provider must be able to provide all of the following procedures:

Carpal Tunnel Syndrome Release (in line with PoLCV Policy)
Excision of ganglions of foot, hand and wrist (in line with PoLCV Policy)
Excision of large painful lipomas, tender sebaceous cysts leading to repeat infection and large/painful infected/irritating warts
Incision and curettage of meibomian cysts
Wedge resection or Zadek’s procedure for painful in growing toe nails
Trigger Finger Release (in line with PoLCV Policy)
Non scalpel vasectomy following pre-operative counseling
The provider can also provide services which are commissioned under the Minor Surgery DES on behalf of practices that are not signed up to the DES, where this is clinically appropriate.

The contract period will be 3 years with an option to extend for a further 2 years
