description
City and Hackney Integrated Commissioning Prevention Workstream are undertaking joint commissioning of two existing services; Social Prescribing (commissioned by the CCG) and 'Community Connections' a community navigation service (commissioned by City and Hackney Public Health).
The new service will aim to provide a service that:
| - | is an accessible and friendly service |
| - | provides personalised support |
| - | is for service users with non-medical needs who are at increased risk of future ill-health |
| - | enables service users to 'navigate' local services |
| - | enables service users to take control of their health and wellbeing and live independently |
The new service will build on the strengths of existing services and be fully integrated into other local provision as part of a Neighbourhood community navigation model including new Primary Care Network (PCN) social prescribing link workers funded by NHSE. Flexibility to respond to developments in community navigation will be a requirement of the service.
