Barnsley Clinical Commissioning Group would like to commission a primary care out of hours service. This will enhance the health and wellbeing of the residents of Barnsley, help decrease health inequalities, and help avoid unnecessary hospital activity.

The aim of the Barnsley OOH service is to:

  • Provide clinical consultations and medical care via a variety of methods, to meet the health needs of patients that cannot be safely deferred to the in-hours period
  • Integrate efficiently and effectively with other services in Barnsley in order to provide a seamless, efficient and caring service which works as part of a greater primary care system
  • Provide high quality, prompt, safe, sustainable and integrated care to the Barnsley population out of general practice hours from the first point of contact onwards
  • Improving out of hospital services so that we deliver more care closer to home and reducing hospital attendances and admissions.
  • The service will afford patients a clinically safe, medically led service that will meet and exceed relevant legal and statutory requirements and Department of Health and local guidance in the delivery of Out of Hours care.
  • The Provider will work with local care providers to ensure that pathways are developed which ensure continuity of care across the spectrum of services available to ensure the patient's transfer from unplanned care to planned care is seamless

Objectives:

  • The service will work towards an integrated service partnership/Alliance
  • The service will evidence a positive patient experience
  • The patient will be signposted to the most appropriate clinician to meet their clinical need
  • The CCG will support the use of a multidisciplinary team that as well as GP's, will include other clinical and non-clinical professionals
  • The service will provide flexible resources to manage peaks and troughs in demand
  • The service will provide Fair equitable access to services on the basis of the need of each individual patient
