The Rapid Access to Carers at End of Life (RACE) Plus will provide direct patient care at end of life. To support a patient and their carer in their home or care home setting, for a maximum of a 48 hour period or 72 hour period over a bank holiday period, during what is perceived to be the final six weeks of the patient’s life or for a patient with a life limiting condition when their caring situation is considered untenable by a health care professional until an appropriate care package can be arranged. This will enable people to die or be cared for in their home setting if is their preferred choice of care, maintaining continuity of care and preventing unnecessary hospital admissions. The overarching aim of the service is to provide a high quality service in end of life care, responding to demand and delivering prompt end of life care on the day of referral via a single point of access with staff trained via an evidenced based End of Life Care programme.
