Support West Cheshire patients with a neurological condition, their carers and West Cheshire Clinical Commissioning Group by:
•Offering a place of care, friendship, advice and support where members and their carers can share the experiences of living with a neurological condition in an informal, relaxed and happy environment. •Offering Physiotherapy and other fitness related services provided by qualified healthcare professionals to ensure access to regular and appropriate exercise for patients with neurological conditions. •Offering education and self-care guidance to support patients in managing their neurological condition.
•Working with partner organisations to provide specialised treatment appropriate to the individual patient, with flexible and responsive step-up/down to the clinical pathway as necessary.
•Reducing the usage of acute services for patients with neurological conditions.
•Reviewing the clinical care pathway for patients with neurological conditions and making suggested changes to the Clinical Commissioning Group.
•Continuing to meet the requirements of the National Service Frameworks and National Institute for Health and Care Excellence guidelines for patients with neurological conditions and their carers.
•Monitoring and evaluating service delivery, supported by patient feedback to ensure the continued delivery of excellent levels of service.
•Reporting the results of patient satisfaction surveys quarterly to confirm the service provision supports patients to make them more able to manage their condition effectively.
•Undertaking training to ensure service provision is in line with current clinical standards.
•Supporting the Clinical Commissioning Group’s goal of reducing hospital admissions by the early identification of health issues and producing patient case studies to show evidence of this.
•Engaging with the Neurological Team at Walton and the Community Integrated Teams in West Cheshire to build and maintain effective working relationships.
•Developing formal step up and step down arrangements for neurological services with the Community Integrated Teams.
•Continuing to develop assessment and individual exercise plans for all West Cheshire patients, to include elements of self care and exercises the patient can complete themselves, away from the community-based setting. •Continuing to show evidence of patient consultation in the development of neurological services.
•Signposting patients, carers and families to further support services and specialist advice, such as Benefits Advisors and Speech and Language Therapy services, as required.
•Making recommendations for treatment to patients, although it is recognised that some patients may choose not to engage in recommended treatments.
•Continuing to direct patients to the Information Point to provide information on their medical condition, self care and how to access wider support and care services, and to give out Carer Information Packs as applicable.
Additional information: Following a 9 month pilot scheme for Provision of Support for West Cheshire Patients with Neurological Conditions and their Carers, NHS West Cheshire CCG now wishes to commission these services and would like potential providers to submit "Expressions of Interest" in providing these services in a community based setting.
This will be a 3 year contract with the option to extend for a further 2 years.
