The Prevention and Enablement Service for Vulnerable Adults, known and referred to as PESVA throughout this document, is a community based service offering a range of interventions including time limited direct support ( up to a maximum of 26 weeks) specifically for younger adults who just fall short of Adult Services Eligibility Criteria, as defined below. The service aims to maximise the long term independence of service users. The service will be delivered in the community and referrals into the service may come from Community Independence Teams, Learning Disability Teams or other representatives of Hampshire County Council Adult Services department, in relation to drop in and telephone support, via self-referral from service users or advocacy services.
Types of service provision will include, but not limited to:

For the purpose of this service, the term ‘Just falls short of Adult Services eligibility criteria’ is defined as: Where an individual is likely to become eligible for a care package within 3- 6 months without intervention, or where an individual experiences a significant event which will, in all probability, result in high use of social care or health services without an intervention.
PESVA will also seek to ensure that people remain active participants in their communities and actively contribute to decisions regarding their support needs.
PESVA will improve the individual’s quality of life, maximise their long term independence and enable them to remain within their community. It will focus on supporting people so they can achieve their optimum level of independence with the lowest appropriate level of sustainable support.
A key aspect of this is approach will be for the service provider to make every effort to work in partnership with a range of statutory, voluntary and community led services to achieve the individual’s identified outcomes. This may include establishing links with local Citizen’s Advice Bureaux, Wellbeing Centres, Job Centres, Hampshire Autism Support Service and other agencies as required.
The service will be divided into three lots and there will be one provider per lot. The lots will mirror the structure of the current Adult Services Learning Disability Directorate Localities (See map):
South East Hampshire (covering Fareham and Gosport, Havant and East Hampshire)
West Hampshire (covering Eastleigh, New Forest and Test Valley)
North Hampshire (covering Winchester, Basingstoke, Deane and Hart and Rushmoor)
