End of Life Care

Sandwell and West Birmingham Clinical Commissioning Group (CCG) has redesigned their approach to End of Life (EOL) services to ensure delivery of an integrated and responsive model which builds on good practice and seeks to innovate and transform the care pathways for patients at end of life.

The CCG is seeking tenders based on a number of EOL services operating in synergy to provide joined up, holistic EOL care for patients. The contract will incorporate the provision of a Co-ordination Hub and Urgent Response Service available 24 hours a day 7 days a week. It will also include provision for specialist palliative care, physical, psychological, spiritual and social support to patients, families and carers. It is anticipated that these services will be provided by a Lead Provider or consortium of providers.

In addition to the services being tendered the CCG also commission a range of specialist Hospital and Community based palliative care services are delivered by Sandwell and West Birmingham Hospitals. These services are part of the block contract and will not be included in the scope of this tender.

Providers must submit their completed ITT by 1st July 2015 at 1700hrs.

The services to which this advertisement relates are for Health Services and therefore remain within the scope of Part B of Schedule 3 of the Public Contracts Regulations 2006 (as amended). Neither placement of this advertisement nor any other indiciation shall be taken to mean that the Contracting Authority intends to hold itself bound by any Regulations, save those applicable to Part B services.
