description
NHS South Cheshire Clinical Commissioning Group (CCG) and NHS Vale Royal Clinical Commissioning Group (CCG) wish to give potential providers the scope to propose innovative solutions to delivering the outcome-based requirements of the community pain management service.
The CCGs expectations are that the service will:
- make a long-term difference to a patient’s (and their carers) ability to cope and live well with pain. Consequently, the service is expected to provide an episode of care, not a long term review of the patient
- adopt the bio-psychosocial model of chronic pain, therefore addressing the patients (and their carer’s) physical, psychological and social needs; associated with pain
- have a team of healthcare professionals capable of assessing and managing the medical, physical, psychological and social aspects of chronic pain (i.e. members of a core team and access to a wider team to support patients with the management of chronic pain)
- use a range of treatments/interventions to ensure care is tailored to the needs of individuals and the people around them (i.e. family, friends, carers)
- will establish good links with primary and secondary care, as well as other community services, to ensure a seamless patient journey. For example, when individuals require specialist procedures or injections for malignant pain and for when patients are discharged back into the care of their GP or for possible onward referral.
- have appropriate levels of resilience across the service that are conducive to the holistic management of pain
- provide adequate access to continued, within-discipline education and development, specific to the area of pain, as well as to broader areas of professional interest.
The contract will be for 3 years with the option to extend for a further 1 year.
