Invitation to submit an Expression of Interest to attend an Open Provider Forum event for Health and Social Care Integration in Cornwall and the Isles of Scilly

Join the conversation to help shape the future of health and care services in Cornwall & Isles of Scilly

Cornwall Council and NHS Kernow and are working with the Council of the Isles of Scilly, Royal Cornwall Hospitals Trust, Cornwall Partnership Foundation Trust, Peninsula Community Health and Kernow CIC to bring all the organisations responsible for health and social care services in Cornwall and the Isles of Scilly closer together.

The objective is to improve health and wellbeing of residents by 2020 to provide seamless health services.

People providing front line services and support in the community are invited to join an Open Provider Forum event hosted in partnership with Healthwatch Cornwall to help co-design the plan for the future.

These events will bring together front line staff and managers representing providers from across health and social care, housing, education, voluntary, public and private sectors in an open forum to work together to develop solutions that will help us deliver better outcomes for people in Cornwall and the Isles of Scilly.
