The Borough Council of Calderdale invites Tenders from those who wish to be considered for the Living Well with Dementia - Community Support, Advice and Information Service. The Provider will provide appropriate information, support, advice and signposting to maximise the benefits to the Individual, their family and Carers from diagnosis to end of life, whether they are living in their own home or in long-term care.
