Home from Hospital Discharge Services

Instead of: Kettering General Hospital NHS Foundation Trust (the "Authority") is issuing an SQ in connection with the competitive procurement of Home from Hospital discharge services. The purpose of the service is to support Service Users and their carers in the period leading up to hospital discharge and during the first 3 weeks following hospital discharge. This supports the Reason to Reside and Home First principles and will help to reduce bed shortages within secondary care through improved patient flow.
