The community stroke support service specified is being commissioned to support provision of community support for stroke survivors and their carers in East Sussex.
The service is being jointly commissioned by High Weald, Lewes, Havens Clinical Commissioning Group (CCG), Eastbourne, Hailsham and Seaford CCG, Hastings and Rother CCG and East Sussex County Council Adult Social Care and Health.
The service will align with the ‘NHS Forward View’ which sets a direction for the future of the NHS including:

| • | Breakdown of barriers in care provision between health and social care |
| • | Supporting a shift from acute to primary and community services. |
