It has been identified that there is a requirement to improve mental health flow outcomes across Surrey and Borders Partnership NHS Foundation Trust (SABP) bed base, with a focus on reducing inpatient length of stay. This is a joint piece of work with SAPB, Surrey County Council will be the lead commissioner.
With a focus on frontline behaviours, practice and ways of working, the improvement work set out in this proposal seeks to ensure patients admitted to inpatient mental health beds are discharged as
early as possible once they are deemed Clinically Ready for Discharge (CRFD) on the appropriate discharge pathway that maximises their independence and likelihood of requiring follow-up emergency or acute intervention.
