description

Bath & North East Somerset Council (on behalf of the Council and Bath & North East Somerset Clinical Commissioning Group) are reviewing their arrangements for brokerage of care home placements (for the Local Authority, Continuing Health Care and Self-Funders) and domiciliary care packages (for Continuing Health Care and Self-Funders).

In order to inform this review the Council are interested in understanding what provision there is in the market to offer a brokerage service for people in need of these placements/packages.

The brokerage service is envisaged to be a team of people who would form a conduit between the commissioner, care provider and hospital/social care teams, ensuring that placements are made in a timely manner, reducing delayed transfers of care from hospital and ensuring that placements in care homes and with care providers meet service users' needs at an acceptable cost.

Please respond to this market engagement request by returning the requested information through ProContract by 18th January 2019. Depending on availability of services in the market the Council may run a market engagement event during February 2019 or make contact individually with any interested providers, to establish what they are able to offer.
