Placement Support and Brokerage Service, West Sussex

Following a further competition under the NHS SBS Patient Discharge Services Framework (reference SBS/18/AP/RIG/9258), Lot 2 - Third Party Brokerage, this notice is to inform the market of the award of a 12 month contract (with an extension option of up to a further 12 months) to Carehome Selection Limited t/a CHS Healthcare for provision of a Placement Support and Brokerage Service in West Sussex for NHS Sussex Integrated Care Board (ICB).

The contract value is £234,000.00 per annum (inclusive of VAT), £468,000.00 for the full potential 24 month term.

The Placement Support and Brokerage Service is an essential service supporting the timely discharge of patients from acute hospitals and community NHS services, supporting urgent and emergency care (UEC) flow through the hospital. The service supports patients who are medically ready to leave the acute trust to find a care package or nursing/residential home placement for those patients who have the financial means to 'self-fund'. The service integrates with local Transfer of Care Hubs and the Local Authority to support the appropriate discharge of these patients.

The service identifies available placements/packages of care on behalf of patients self-funding their ongoing care needs (Self Funders) by supporting the identification and negotiation of residential/nursing home placements or packages of care to enable timely discharges from acute hospital beds.

Services will be provided to individuals who are either residing within the West Sussex County Council local authority boundary or individuals who are registered with a NHS Sussex Integrated Care Board (ICB) General Practitioner (GP).
