' Young people moving out of foster care or residential provision whom may not have complex and challenging behaviours but do have needs around separation, loss and trauma and associated behaviours requiring support to enable them to prepare for independence. Young people may present as homeless and require short-term accommodation whilst a Joint Housing Assessment is being undertaken. Young People who are homeless and/or require lower level of support to maintain their accommodation with a minimum package of support. Support Needs: Group living/ shared accommodation; supported tenancy with a low level of floating support required. Hours of support will be based on individual needs and circumstances of the young person. or Floating support only, if the young person is has already moved into independent accommodation or if accommodation is provided elsewhere. Ability to transfer tenancy to young person for the accommodation within housing benefit limits.'
