description
Increasing health checks in the borough for those eligible residents aged between 40 – 74 years old. The provider will also need to provide a patient-centred triaging and assessment service that will triage individuals with risk factors for cardiovascular disease or diagnosed diabetes or who require cardiac phase 4 services in the community and refer to evidence based behavioural change services some of which will be delivered in-house by the provider.
