NHS North West London ICB requires the provision of an integrated consultant-led community respiratory service in Hounslow. The ICB is looking to tender this opportunity via a closed below threshold procedure via the Atamis e-procurement portal which will be launched in the next few weeks.
Key requirements include provision of a: -Pulmonary Rehabilitation Service
-Home Oxygen Assessment Service
-Community-based specialist Nursing & Consultant Service -Chest Rehabilitation
Objectives of the service are to:
- Provide an integrated care service where the Provider will work with other services/stakeholders, to ensure continuity of care is provided to the patients with chronic respiratory conditions, e.g. Chronic Obstructive Pulmonary Disease (COPD)
- Ensure pulmonary rehabilitation is accessible to all individuals with chronic respiratory conditions e.g. COPD
- Provide a systematic & integrated assessment & review of patients on home oxygen (HOS AR)
- Provide recently discharged COPD patients with support so that they can self-manage their condition & avoid re-admission
- Reduce non-elective admission to hospital for those patients with diagnosed COPD, i.e.
-Through advising primary care (General Practices) on the early identification of deterioration at primary care level
-Following-up identified discharged patients to promote health & improve self-management
-By facilitating referral into Pulmonary Rehabilitation, oxygen services & other related services as appropriate
-By proactively identifying the COPD patients either via SystmOne (S1) search or using WSIC dashboard & offering support to manage any exacerbation
- Empower patients & carers to self-manage their condition & make informed choices through improving their knowledge & education provision as required
- Provide a chest physiotherapy service for patients with chronic respiratory conditions
- Provide a service to patients over 18 years of age & registered with Hounslow GPs
The ICB is committed to assessing the needs of the service users & to consider how it needs to secure the needs of the people who use the services & improve the quality & efficiency in the provision of the services including through the services being provided in an integrated way.
The service is required in the borough of Hounslow for a twelve (12) month period plus the option to extend for a six (6) month starting 1st October 2023 with a maximum affordability contract value of £312,686 inclusive of VAT in Year 1 & £163,976 inclusive of VAT in Year 2. A mobilisation period will be applied prior to the commencement of the start date. The NHS Standard Contract (short-form 23/24) will be used.
